Emergency Information

Who may pick up your child at the ending of VBS Camp?

If you notated your child needs medicine while participating at Vacation Bible School read below: Note that if your child is under any medication that will need to be administered while they are at Vacation Bible School, we cannot administer medication. You will need to assign a parent or legal guardian who can come administer the medication. If this is the case, please note who this person will be for our records, at the time the medication will need to be administered so that we can coordinate with your child's Crew Leader.

COVID-19 SPECIAL PROGRAM ATTENDACE PARENTAL ACKNOWLEDGENT AND DISCLOSURE

Each Statement needs to be initialed by Either parent or guardian.

I understand that in order to attend the program my child must be free from COVID-19 symptoms. If, during the day, any of the following symptoms appear my child will be separated from the rest of the people in the center, in a supervised, secure area. I will be contacted, and my child MUST be picked up from the facility within 1 hour of being notified.

Symptoms include:

-Fever of 100.0 degrees Fahrenheit or higher

-Dry Cough

-Shortness of breath

-Chills

-Loss of taste or smell

-Sore throat

-Muscle ache

-Any other symptom identified by CDC as associated with COVID-19

While we understand that many of the symptoms can also be related to non-Covid-19 related issues, we must proceed with an abundance of caution during this public health emergency. These symptoms typically appear 2-7 days after being infected so please take them seriously. Your child will need to be symptom-free without any medications 72 hours before returning to the facility.

Waiver of Liability*

In consideration of my participation in the "Life Walk Ministries Vacation Bible School" event. BEFORE,  DURING, AFTER EVENT, I hereby waive and Release Parties, and all of their representatives, employees, directors, agents, volunteers, insurers, independent contractors, successors, property, and assigns, harmless from any and all liability for actions, causes of actions, suits, contracts, torts, claims inequity, discrimination, demands, promises, promises, expenses, and damages on account of injury to myself or my property, including but not limited to injury resulting in death, which I have or which may arise in the future in connection with the activity or participation in any other associated activities. I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by law of the State of Florida, with the venue for any dispute being in Hillsborough County, Florida, and that if any portion thereof is held invalid, it is agreed that the balance shall notwithstanding continue in full legal force and effect. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not mere recital.

MEDIA RELEASE & LIABILITY RELEASE *

By signing below, and for the good and valuable consideration of participating in an event hosted by the Life Walk Ministries (the "Released Parties"), I hereby permit the Sponsors to record, by writing, video, photographic, or audio recording device, or by any other analog or digital means, my actions, physical likeness, biographical information, and/or voice. Additionally, I hereby grant the Sponsors without royalty or other compensation now or in the future, all rights of every kind and character whatsoever, in perpetuity, in and to any such recording, along with any additional recording I night provide to the Sponsors and any benefits inuring to the Sponsors as a result of its use of any of the foregoing recordings. Among other things, the Sponsors may, but are not required to, copy or reproduce the recording. Among other things, the Sponsors may, but are not required to, copy or reproduce the recording, edit or modify it, incorporate it into another work, display or broadcast it or any of the foregoing privately or publicly, and use or license it or any of the foregoing for use by others, all for the sole benefit and at the sole discretion of the Sponsors for the advancement of the Sponsors charitable purposes. All permissions granted herein extend to any successor or assign of Sponsors and bind me and my heirs, successors, and assigns. I hereby release and discharge and agree to hold harmless the Sponsors, and their respective representatives, employees, directors, agents, volunteers, insurers, insurers, independent contractors, successors and assign, from any claims or damages, including but not limited to defamation or violation of the recordings. This release shall be construed, interpreted, and governed in accordance with the laws of the State of Florida, with the venue for any dispute being in Hillsborough County, Florida, and should any provision of this release be determined invalid, such invalidity does not affect any of the remaining provisions of. I am fully competent and have the right to contract in my name.

COVID-19 RELEASE *

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN LIFE WALK MINISTRIES Summer Camp, Including but not limited to any risk that might arise from negligence or carelessness on the persons or entities being released. because of their possible liability without fault, or from danger to any real or perceived exposure to COVID-19 virus through participation in this event and future events held by LWM and not limited to outside including events held inside any buildings as well.  I certify that I am physically well, I do not have any symptoms of COVID-19, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity, and or future events by LWM.  A. I WAIVE, RELEASE, AND DISCHARGE, LWM and/or their directors, officers, employees, volunteers, representatives, and agents from any liability, including but not limited to, liability arising from negligence or fault of the agents of LWM or other participants of this event, for any real or perceived exposure to COVID-19 Illness, death, disability, personal injury, property damage, or actions of any king wich hereafter occur to me including my traveling to and from this activity.  B. INDEMNITY, HOLD HARMLESS AND PROMISE NOT TO SUE the entities or persons mentioned in this Waiver and Release From Liability form from any all liabilities or claims otherwise. I acknowledge that they are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf and they are not responsible for any real or perceived exposure to COVID-19.  I acknowledge that this activity and or future events may involve direct physical contact with individuals other than my immediate family and/or household members. I acknowledge that I am attending this event and or future events of my own free will and LWM and/or their directors, officers,employees, volunteers,representatives, and agents did not force or compel me to attend. I acknowledge that I have been to public places, businesses, and/or been in the presence of individuals other than those in my household before and will be again after the LWM event.  This Waiver and Release from Liability Form shall apply to all minors in my legal care, custody, and control attending this event. This Release from Liability shall be construed broadly to provide a realise and waiver to the maximum extent permissible under applicable in the State of Florida.  I CERTIFY THAT I HAVE READ THIS DOCUMENT (Release and Waiver of Liability Form) AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL AND UNDERSTAND FULLY THE RIGHTS I AM SURRENDERING THIS RELEASE AND WAIVER OF LIABILITY FORM.
